"Your offspring" doesn't necessarily mean *all* of Avraham's offspring. Avraham had six more sons later in life (Genesis 25:2), but nobody considers them heirs to the land either. Yishmael and the other six were specifically disinherited; Avraham gave everything to Yitzchak (Genesis 25:5). After Avraham died, God blessed Yitzchak (Genesis 25:11) but there is no mention of Yishmael. In Genesis 21:12 God tells Avraham "it is through Yitzchak that offspring shall be continued for you". God's reward to Yishmael was that he would be the father of a great nation (Genesis 21:13). Thus, both God and Avraham designated Yitzchak as Avraham's heir.
